If you are or were employed by Elite Medical Transportation, LLC, and had unauthorized, adverse deductions made from your pay between August 16, 2013, and November 13, 2025, you may be Class Member and entitled Settlement benefits.

To receive benefits, Class Members must submit a valid Claim Form by March 9, 2026.

For more details regarding the Settlement benefits, please review the Settlement Agreement, and Notice.

To start your online claim, you can find your Notice ID & PIN printed on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator at (888) 369-3780. Please request your ID & PIN by providing the Class Member’s full name and mailing address.

You may also complete and return the claim form enclosed with the Notice by U.S. mail to:
